SPI - Pathway to Certification in Sonography

Several national organizations offer certification exams for ultrasound professionals, with the American Registry for Diagnostic Medical Sonography (ARDMS) being the most widely recognized. Other major certifying bodies include the American Registry of Radiologic Technologists (ARRT) and Cardiovascular Credentialing International (CCI).

1. American Registry for Diagnostic Medical Sonography (ARDMS)

ARDMS offers credentials based on a core physics exam and specialty-specific exams. To become fully certified, you must pass both the Sonography Principles and Instrumentation (SPI) exam and a specialty exam within five years.

Required exams:

  • Sonography Principles and Instrumentation (SPI) exam: This assesses your knowledge of the fundamental physics and instrumentation of sonography. It is a prerequisite for earning most ARDMS credentials.
  • Specialty exams: After passing the SPI, you must pass a specialty exam in your chosen field.

Options include:

  • Registered Diagnostic Medical Sonographer (RDMS): For general sonography specialties like Abdomen (AB), Obstetrics & Gynecology (OB/GYN), and Breast (BR).
  • Registered Diagnostic Cardiac Sonographer (RDCS): For cardiac specialties like Adult Echocardiography (AE).
  • Registered Vascular Technologist (RVT): For vascular technology (VT).

2. American Registry of Radiologic Technologists (ARRT)

The ARRT offers sonography credentials as part of its broader focus on medical imaging.

  • Registered Technologist in Sonography (R.T.(S.)): This certification covers a broader range of general sonography practices.
  • Other options: The ARRT also offers certifications in Vascular Sonography and Breast Sonography, often as a post-primary pathway for those already certified in a related field.

3. Cardiovascular Credentialing International (CCI)

CCI specializes in certification for cardiac and vascular specialties. Some of the credentials offered include:

  • Registered Cardiac Sonographer (RCS)
  • Registered Vascular Specialist (RVS)

General requirements for certification

To become a certified ultrasound professional, you must meet educational requirements, gain clinical experience, and pass the necessary exams.

  • Complete a sonography program: The most common pathway is to graduate from an accredited program, usually an associate or bachelor's degree. Accreditation by the Commission on Accreditation of Allied Health Education Programs (CAAHEP) ensures the program meets national standards.
  • Gain clinical experience: Programs typically include hands-on clinical training. A certain number of hours or documented procedures are required to be eligible for certification exams.
  • Pass certification exams: You must pass the required exams from one or more of the certifying bodies mentioned above.
  • Get certified: Due to the growing need for quality performance and competitive nature of the industry, most employers prefer to hire Registered or Certified sonographers. It is therefore recommended that acquisition of relevant certifications be imperative for job placement and security. Success in the SPI Examination is either a prerequisite to, or an integral component of the board exam.
  • Maintain certification: Keeping your credentials valid requires meeting continuing education (CME) requirements and paying annual fees.
